Follow up Phone Calls Example

Hey team,
      Eric asked me to write an email with reports on our follow-up calls to patients. It has been very instructive. Here is what we have so far:

Jeremy Scott - 17 y/o with cellulitis of the forearm - He did not fill his Clinda script because the pharmacy said it would cost $500 and he did not know he had insurance. His arm is feeling better with no pain or swelling but is still somewhat red. I gave him his insurance info, looked up doctors for him to call, and got permission to call his probation officer who agreed to remind Jeremy to get a PMD.

Walter Guevara Vanegas - 4 d/o M with jaundice - Parents have been living in a house with no heat or AC for the last several months and the landlord refuses to fix it. I called BMS and got into contact with Health Leads, who has a social worker who has agreed to take on the case. The father was appreciative of the help and is looking forward to the call. I added the Health Leads # to our signout.

Peyton Byrd - 2 w/o with enterovirus positive meningitis - Spoke to Mom, pt has afebrile, has been eating well, Mom had no questions. 

Jeremiah Alston – 7 m/o with bronchiolitis kid - Mom upset because she felt that they were sent out before she was ready. Mom was scared because his breathing continued to sound bad. He was set up for a pediatrician appointment and a home visit from nursing. 

Phillip Lewis - 8 y/o quadruplet male hx CLD who p/w abdo pain and intolerance of PO - spoke to Mom, who said that he has been responding to Zofran and able to take more PO fluids than he vomits, already saw PMD

Donna Pratt - 16 y/o F with pyelo/PID/BV, discharged on Doxy and Flagyl - spoke to patient who continues to report intermitting nausea and vomiting, metallic taste in her mouth, and flank and lower abdominal pain. The pain is improved from before, but still present. I counseled the patient to take the Flagyl with food and return to the ER if her pain worsens

Sky Christenson - 3 y/o M p/w abdo pain and dehydration s/p gastroenteritis + parents divorcing - no one picks up phone

Kayla White - 18 y/o F with peritonsillar abscess - called to follow-up, patient much improved on Clindamycin, very appreciative of call

Michael Lang - 7 m/o M with severe croup - parents initially unable to fill the Clindamycin script due to no concentration being written on it or the Cefpodoxime script due the pharmacy not having it. Pt also written for very high 4 mg/kg/day dose of Prednisolone. We called the pharmacy to clarify the dosage of Clinda and Prednisolone and the parent was able to fill the scripts. Mom told to give 2 mg/kg/day Prednisolone then 1 mg/kg/day prednisolone and not to fill the Cefpodoxime. Pt is much improved, Mom thankful for call.
